10. I hereby give permission for images of my children and my family, captured through video or still photos to be used solely for purposes of S.E.E.K. related online and printed material and publications and waive any rights of compensation or ownership thereto. (1 required) I give permission.

Section 111 PA School Code: I attest that no adult living in the home or no person having legal custody of the listed children have been convicted within five (5) years of today’s date of any of the criminal offenses “enumerated in subsection (e) of section 111 of the school code and are not listed as a Megan’s Law offender.” (1 required) I attest
13. The Pennsylvania Legislature passed Act 153 & Amendments, amending that the state Child Protective Services Law to impose greater background check requirements for all individuals with direct contact with children (which includes all parents that attend co-op regularly).

Payment Instructions

The SEEK Membership Fee is $95/year, divided into two amounts and collected the following times:

$60 due during Class Registration (April) at our Payment Day (during co-op hours)
$35 CASH ONLY due during the August evening Kick-off Meeting
