MEMBERSHIP AGREEMENT

Liability Release: The Home-school Life system is being updated to accommodate the signing of liability release waivers digitally. A digital waiver for each parent and child must be signed by August 1.

I understand that membership is for me, my spouse, and children living in my home. If there is a parent who lives in a different household, they are required to apply for their own membership. 9. I hereby understand the terms of membership in CCH and agree to follow these guidelines for the entire duration of my membership. I also understand that failure to comply with these terms at any time could result in membership termination, and that this form is binding for as many years as I choose to re-enroll.
