Livescan Day 2
Every teacher and primary parent/guardian attending Fridays must have a one-time live scan background check.
- You must sign up for a specific time slot. (There are 4 slots every 15 minutes.)
- Print the paperwork (click File 1 below to download), and have it filled out when you arrive for your appt.
- Fill out only the area marked in yellow under 'Applicant Information'
- Bring your drivers license
- Cost is $19 (cash, check, card or Venmo). If the cost is a hardship please let Andrea Patton know before your appt and the church will pick up the fee.
- We will have 2 days for appointments. If you cannot make these days you will have to make arrangements with www.blueoakmobile.com and the cost will be $35.
- If you have a highschooler under 18 driving themselves on Fridays, and is volunteering on behalf of a parent, this will not apply to you.
- Livescan is only good at the agency you are volunteering with and cannot be transferred. So, if you've had one previously with a different organization you will have to do another for Anchor.
