Required Membership Information for Labyrinth

All memberships are pending approval of the Labryinth Board of Director. Completing this registration form does not constitute enrollment in Labyrinth Co-op. If you are not currently a member of the Labyrinth Co-op, please contact the Prospective Family Coordinator (info@labyrinthcoop.org) to inquire about openings, current wait lists, and the registration process.

The following 4 forms must be completed in order to enroll for 2025-2026 classes. Failure to complete these forms will result in delayed class enrollment for your family. 2. Follow this link to open a pop-up window to read and sign the Labyrinth Disclaimer of Liability. (1 required) I have signed the Disclaimer of Liability.
3. Please follow this link to read to a pop-up window to sign all 9 sections of the Labyrinth Parent Contracts. (1 required) I have signed the parent contracts.
4. Please follow this link and either upload a copy of your driver's license or confirm with your digital signature that you will present it in person before Class Enrollment. (1 required) I uploaded my license or will provide it in person
