Waiver of Liability

This agreement releases Brookhaven Home Educators (BHE), Participating Organization or Church, Etc., from all liability relating to injuries that may occur during Game Night, Co-Op, Field Day, Etc. By signing this agreement, our family agrees to hold BHE, Participating Organization or Church entirely free from any and all liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

We swear that we are participating voluntarily, and that all risks have been made clear to us. Additionally, we do not have any conditions that will increase our likelihood of experiencing injuries while engaging in this activity.

By signing below, we forfeit all rights to bring a suit against BHE, any Participating Organizations, or Church, Etc. for any reason. We will also make every effort to obey safety precautions as listed in writing and as explained to us verbally. We will ask for clarification when needed.
