We, the parents or legal guardians of the above children hereby grant permission for the foregoing children to participate in physical activities at Jennie Dean Park, All Saints Church in Manassas, Virginia, and at such other locations as may be designated from time to time under the auspices of Manassas Area Roman Catholic Homeschoolers (“MARCH’).

Further, on behalf of ourselves and the above listed children, we do further agree to indemnify, defend and hold MARCH, and any individuals supervising such activities on behalf of MARCH, harmless of any claims, damages, injury to person or property arising directly or indirectly out of such activities.

Payment Instructions

Please submit a check for the membership fee of $30 for the year along with your liability form to Veronica Perrington (address on form). Instructions will be provided by the MARCH Treasurer.
