Registration Form

There are several pages of forms for you to complete in order to have your registration finalized. Please complete everything in this series before you go on to register your children for their 2024-2025 Forge classes. 

Please understand that your registration forms, background check, and facilities fee of $60 must be submitted in order for your registration to be complete.  If your forms have not been completed by Tuesday, April 30th 2024 and facility and class fees have not been paid by June 15th 2024 your children may be dropped from the classes you have placed them in. This is very important for you to understand.  Instructions on how to pay fees are forthcoming.

We ask that both parents be present and go through this registration process together. Initials from both parents will be required in certain areas.

All info on this website is for the confidential use of Forge and its members.  Memorize or write down your user name and password in a secure place.  At no time should your User Name and Password be given to anyone else.
