  • Read over, sign and return 
• Application
  • Fill out and return with $25 fee
• Background Check Form
  • Fill out for each adult that will accompany your child to CHARA. CHARA will run background checks for each parent. If there will be any other adult, at any time, accompanying your child to co-op, please fill out the form, and return along with a $10 background check fee.
• Enrollment Contract & Photo Release Form
  • Sign and return
  • Fill out and return stating whether or not you would like photos of your child on our social media page.
