1. For "Parent On-Site" please enter the information for the parent who will be attending co-op weekly.

2. Please store the username/password you select, as this is how you will log in to the website in the future.

3. At the top of the large grey "Child Information" box, please use the   button to add additional children to your form.  Please include all children who will be attending co-op, including nursery age.  If you are expecting (congratulations!), please include them as well using the due date in the birthdate field.  This helps us to plan our nursery room for the year!

Co-op Costs 2025-2026

Student Supply Fee

     $50 (half-day)  OR  $75 (full-day) per student (excl. nursery) covering the full year

     Supply fees are the minimum needed to run the co-op, and therefore we are unable to offer multiple-children discounts.

Donation to Sts. Simon & Jude Parish

     $75 (minimum) per family donation to Sts. Simon & Jude Parish

Registration & Technology Fee

     $30 per family registration & technology fee

Additional Possible Costs

     Additional costs such as course, lab, and/or textbook rental fees may apply for certain classes.  Purchase of textbook and/or supplemental resources may be required, particularly for core courses.
