Requirements for Completed Enrollment:

Fill out all the information in this form, including application information for EACH child that will be enrolled.
Pay the application fee of $100 (for one child) or $150 (if more than one child) (due at the time of application).
Send a copy of any report cards & previous achievement test scores to cornerstoneclassicalacademy@gmail.com.

Once you have completed these steps, your application will be reviewed by our administration.

Please allow up to one week for application review.

Upon approval of your application, you will receive a Member Login.

This login will grant you access to:

PHOTO USE RELEASE: I hereby grant and authorize Cornerstone Classical Academy the right to take, edit, alter, copy, exhibit, publish, distribute, and make use of any and all pictures or video taken of my child by Cornerstone Classical Academy to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration.

This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing. I understand and agree that these materials shall become the property of Cornerstone Classical Academy and will not be returned. I hereby hold harmless, and release Cornerstone Classical Academy from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate. I have read this release before signing below and I fully understand the contents, meaning and impact of this release.

*Note: NON-REFUNDABLE STATUS OF TUITION FEE: Due to expenses forecasted and incurred (regardless of any reduction in student population), and in consideration of the per-student cost for materials and overhead for the year, we cannot refund any paid registration or tuition fees. Therefore, I understand my financial commitment and agree to pay the tuition for the entire school year even if my child does not complete the school year.
