3. CHALC/Adventure Club Participation Agreement
CHALC Group: Adventure Club , School Year: 2024 - 2025, Location: Field Trip Group - locations will vary
This Agreement is between the student indicated above (“the Student”), the parent(s)/guardian(s) of the student indicated above, and the Coalition of Homeschoolers Across Lancaster County (“CHALC”) and the CHALC member group (Adventure Club) indicated above. In consideration for the opportunity for the Student to participate in regularly scheduled Adventure Club activities during the school year indicated above, and in addition to any fees paid on behalf of the Student, the undersigned acknowledge and agree to the following:
We acknowledge that participation in Adventure Club or CHALC activities involves risk of injury and/or illness to the Student, including but not limited to sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage.
We acknowledge and accept such risks associated with both participation in and transportation to/from Adventure Club or CHALC activities.
We accept personal financial responsibility for any injury or other loss sustained during participation in and/or transportation to/from Adventure Club or CHALC activities, including but not limited to medical treatment rendered to the Student that is authorized by CHALC, Adventure Club, or their respective representatives.
We agree to indemnify, defend, and hold CHALC, Adventure Club, and their respective representatives for any injury or other loss sustained or inflicted by the Student arising out of participation in and/or transportation to/from Adventure Club or CHALC activities, whether or not such injury or other loss arises, in whole or in part, out of negligence of CHALC, Adventure Club, and their respective representatives.
Your $40 membership fee pays for your CHALC membership, use of the website and Adventure Club costs. This covers the school year from July 1, 2024 - June 30, 2025. We do not pro-rate membership costs if you join later in the calendar year. By typing your digital signature below, you agree and acknowledge that this $40 fee is non-refundable should your family choose to withdraw before the calendar year is complete. If requested, we can remove your account from recieving Adventure Club emails or forum posts and from the website directory should you decide to end your membership prior to the end of the school year.
Payment instructions will be emailed after registration is complete.
