1. Service Hours Agreement
I understand the RHSA is a cooperative organization that requires my fulfillment of  1 hour per week for 32 weeks
of service hours throughout the year,. In lieu of service hours, I can submit a $300 payment,
at the time of registration. I understand that my failure to fulfill the Service Hours Agreement
will result in the forfeiture of the opportunity to register for Co-op the following year. Families
with students in junior high and high school classes can have the student sign up for a service
job.

4. Payment of tuition
I understand that by enrolling my child/children in the RHSA Teaching Co-op, I am
guaranteeing that I will pay all monthly tuition in accordance with the payment schedule. If I
withdraw my child/children before the end of May or if my child is dismissed from a class or

for that class.

shorts shorter than 5 inches above the knee. Yoga pants, leggings and the like must have a shirt
that covers the bottom. No undergarments, midriff, skin tight or see-through clothing allowed. If

Our academic policy is in place to uphold the integrity of our classes. Students are required
to submit work that is complete, legible, on time and original. Late work will not receive full
credit. Cheating is not tolerated. Please see the full academic policy in the handbook.

courtesy to our teachers, please attempt to work things out with the teacher when possible. If

SERVICE HOURS REQUEST FORM

Each family is required to contribute service hours throughout the year to make this co-op a
friendly, well-organized place to learn. The commitment is 1 hour each week for 32 consecutive
weeks, not including holidays. Service hours correspond to class times. There are other areas to
serve within the Co-op that count for service hours. Some of these jobs take a little more time
each week and are a year-long commitment.
For those who cannot serve, payment in lieu of service is available. This is a one-time payment
of $300 due at registration. A limited number of payment spots are available.
If you are unable to serve any of your weeks and do not find a substitute to take your place, there
is a fee of $15 you will owe to RHSA to cover your missed week. You are required to bring this
fee the next week you return.
Families with children older than 6th grade and thus not on campus, may elect to have their
student sign up for a service job.

Release of Liability and General Agreement

In consideration of being permitted to participate in the Richardson Home School Association
Teaching Co-op,
(Please type below: first name of each parent or legal guardian, each child in the family, even
those not attending Co-op, and any other family member who may be likely to come on
campus.)

(hereinafter referred to as “Family”) agree to the following:
1. Release, waive, discharge, and covenant not to sue either the RHSA Teaching Co-op or Prairie
Creek Baptist Church, their officers, employees, board members or other families participating
in the Teaching Co-op of RHSA (hereinafter referred to as “Releasees’) from all liability to
Family for any loss or damage and any claim or damage on account of injury to the person or

property or resulting in death of a Family member while Family is participating in Teaching Co-
op of RHSA activities or any other activities on the premises of Prairie Creek Baptist Church.

2. Family agrees to indemnify Releases from any loss, liability, damage, or cost Releases may
incur due to the presence of Family in or on the premises of Prairie Creek Baptist Church.
Family realizes transportation to and from Co-op or any time off campus is the Family
responsibility.
3. Family assumes full responsibility for and risk of bodily injury, death, or property damage
while in or on the premises of Prairie Creek Baptist Church and/or while working or for any
purpose participating in the activities of the Teaching Co-op of RHSA.
4. Family expressly agrees that this release, waiver, and indemnity agreement is intended to be
as broad and inclusive as permitted by the laws of the State of Texas and that, if any portion of
the agreement is held invalid, it is agreed that the balance shall, notwithstanding, continue in
full legal force and effect,
5. Family, in consideration of being permitted to participate in the Teaching Co-op of RHSA, for
Family, and Family's heirs, executors, administrators, and assigns, releases and forever
discharges all Releasees, and their heirs, administrators, and executors of and from any and
every claim, demand, action or right of action, of whatsoever kind or nature, either in law or in
equity arising from or by reason of any bodily injury or personal injuries known or unknown,
death or property damage resulting or to result from any accident which may occur as a result of
participation in the Teaching Co-op of RHSA or any activities in connection with the Teaching
Co-op of RHSA.
6. Family states that both Mother and Father of Family or legal guardians have carefully read
the above release and know the contents of the release and sign this release as Family's own free
act.

7. Family releases all officials and professional personnel from any claim whatsoever on account
of first aid, treatment or service rendered to the Family during participation in the Teaching
Co-op of RHSA.
8. RHSA and the Teaching Co-op operate under biblical guidelines and believe that the Bible
commands them to make every effort to live at peace and to resolve disputes with each other in
private or within the Christian church (see Matthew 18: 15-20; 1 Corinthians 6: 1-8). Therefore,
the parties agree that any claim or dispute arising from or related to this agreement shall be
settled by biblically based mediation and if necessary, legally binding arbitration in accordance
with the Rules of Procedure for Christian Conciliation of the Institute for Christian
Conciliation. The parties understand that these methods shall be the sole remedy for any
controversy or claim arising out of this agreement or any dispute arising out of any activities or
events associated with the Teaching Co-op of RHSA and expressly waive their right to file a
lawsuit in any civil court against one another or against another RHSA Teaching Co-op family
for such disputes, except to enforce an arbitration decision.
9. This agreement shall be binding on each member of the Family, their personal
representatives, assigns, heirs, and next of kin.
10. This release contains the entire agreement between the parties to this agreement and the
terms of this release are contractual and not a mere recital.

RHSA Photography Release Form

This form grants Richardson Home School Association permission to use photographs and/or videos taken during co-op activities, events, and field trips for marketing, social media, fundraising, and other promotional purposes.

Permission to Use Photographs and Videos

By signing this release form, you (the parent/guardian/teacher) authorize RHSA to use photographs, videos, or other images of you and/or your child(ren) taken during co-op activities. These images may be used in the following contexts:

  • On RHSA's website
  • In print or digital marketing materials (e.g., brochures, newsletters, flyers)
  • On social media platforms (e.g., Facebook, Instagram)
  • For fundraising campaigns
  • For promotional materials related to RHSA’s activities and events

These images may be altered, cropped, edited, or otherwise modified as necessary for the intended purposes.

You understand and agree that no compensation will be provided for the use of these images, and no further approval or payment will be required before their use.

Participation is entirely voluntary, and you may choose not to grant permission for images of you or your child(ren) to be used in this manner. Choosing to not participate will not affect your family’s participation in co-op activities or your relationship with RHSA.

Character Code of Conduct Agreement

Richardson Home School Co-op

Purpose:

As members of Richardson Home School Association, we aim to create a positive, loving, and Christ-centered environment where students can grow in knowledge, character, and faith. This Code of Conduct outlines the values, behaviors, and expectations we uphold as a community. By signing this agreement, you pledge to respect these standards, honor one another, and reflect Christ’s love in all interactions.

I. General Expectations of Behavior

1. Respect for Others

  • Treat all students, teachers, and staff with kindness, courtesy, and respect
  •  Be mindful of personal space, feelings, and differences in others; listen attentively when others are speaking, and speak thoughtfully

2. Honoring Authority

  • Respect the authority of co-op leaders, teachers, and any adult or designated volunteer
  • Follow instructions promptly and willingly

3. Christian Values

  •  Strive to live out the values of love, kindness, patience, humility, forgiveness, and respect as taught in the Bible
  •  Use words and actions that reflect the teachings of Christ
  •  Showing love and concern for others’ well-being

II. Behavior Standards

1. Bullying 

  • Bullying of any kind—physical, verbal, or social—is strictly prohibited. This includes teasing, mocking, exclusion, intimidation, or making fun of others.
  • Treat every person with dignity, as each is made in the image of God.
  • Report any bullying behavior to a teacher, staff member, or parent volunteer immediately.

2. Respectful Language and Actions

  •  Use words that are uplifting, kind, and encouraging to others. Avoid harsh, hurtful, or offensive language.
  • Swearing, cursing, or using disrespectful language is not allowed.
  • Pushing, hitting, kicking or throwing objects at others is strictly prohibited. 
  • Apologize and seek forgiveness when you have hurt or offended someone.

3. Honesty and Integrity

  • Be truthful in all your words and actions.
  • Stealing, lying, cheating, or misrepresenting the truth is not acceptable.
  • Practice integrity in schoolwork, assignments, and interactions with others.

III. Respect for Property

1. Care for PCBC Property

  • Treat the facilities, supplies, and resources provided by PCBC and RHSA with care and responsibility.
  • Return borrowed items in good condition.

2. Respecting Personal Property:

  • Always ask permission before using someone else’s belongings.
  • Return personal items promptly and in the condition they were received.

IV. Conflict Resolution

1. Resolving Disagreements 

  • If you have a disagreement with another student, try to resolve it in a calm, respectful manner.
  • If necessary, seek assistance from a teacher, staff member, or parent to help mediate the situation.

2. Forgiveness and Restoration

  • If you wrong someone, seek their forgiveness and make an effort to restore the relationship.
  • If you are wronged, be quick to forgive, just as Christ has forgiven us.

VI. Consequences for Misconduct

  • Violations of the Code of Conduct will be addressed with a spirit of grace and correction, with consideration for the severity of the issue.
  • Possible consequences may include:
  1.  A verbal warning
  2. Temporary removal from an activity
  3. An incident report
