Parent Contract

I (name), a participating member of CHRISM Co-op, agree  to come to co-op on time on scheduled Fridays, and to fulfill the  commitment for which I have volunteered for this school year. I also agree to fulfill my duties wherever I am placed in order to help leave the facilities better than we found it. I will  make every effort to notify the Director of any absences before the  CHRISM co-op day begins.  

I understand that if a problem arises I will speak directly to the person  involved and/or speak with the Leadership Team to resolve the matter as  quickly as possible. By checking the box during the membership process, I confirm that I have read this agreement, I accept all terms laid out in the Policy and Procedures Form and in the Student Contract. 

I have received and agree to the meeting dates, times, class offerings and  schedule of fees and pledges. 

I understand that lack of acceptance to these terms will be viewed as  withdrawal from membership.
