3. If you are not teaching a class, you will be assigned to be a helper. Helpers serve 2 periods each school day and have 1 period off when possible. The following questions will help us match helpers with classes and teachers. Which age group(s) would you prefer to work with?

I (we) being of 21 years of age or older, do hereby release Cornerstone Co-op, Trinity Baptist Church and its directors, employees and agents from any and all liability for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever while the said child/children is/are participating in the Cornerstone Co-op. Furthermore, the undersigned agrees to release Cornerstone Co-op, Trinity Baptist Church and its directors, employees, and agents from any liability for negligent, willful or intentional acts by participant that should cause injury or death.

I agree to complete and turn in to the Registration Coordinator at my earliest convenience. I understand that every adult at Cornerstone Cooperative must consent to a background check. I understand that my registration fees include payment for 2 background checks, and if additional background checks are necessary for my family's participation, I will be invoiced $7.50 per background check.
