NOTE: If you have already been a member of ECCHO and are just "parked" in order to pay dues for the new year please do NOT fill out this form. Go back to the previous page and click the 'Pay ECCHO Dues and Class Fees' link in the bottom of the left column.
Please read our Statement of Faith, Constitution and Bylaws, Child Protection Policy, Code of Conduct, and Dress Code before proceeding with registration.
To Join ECCHO please complete these 3 steps:
1. Fill out the membership application below.
2. Submit background checks for family members (18+) who will be attending ECCHO events. (Parents, Grandparents, Siblings, Etc.) These can be emailed to rebecca@eccho.us or filled out/turned in at the New Family Open House event.
3. Sign up and attend a New Family Open House - you will find the next available one on the ECCHO calendar. We plan this every three months (or sooner) as needed.
Family dues can be paid by cash, check, or Zelle (zelle@eccho.us) at the New Family Open House.
After completing the application form below, including the medical, liability release, and emergency contacts pages, please complete registration by paying the membership dues, and submitting the background check form (please fill out and submit to rebecca@eccho.us).
Please note that membership will not be approved until all steps are complete.
